If you are due to undergo your surgery at the Eye & Ear University Hospital, you must first have a medical evaluation with your Doctor. Your Doctor will transfer your medical file to the Scheduling Office, in order for you to decide on a convenient date for your operation.
You will be asked by the Scheduling Office to present all required documents depending on your third party guarantor.

Private Insurance:
You should present the following information and documents to the Scheduling Office.
- Valid Insurance Card
- ID Card
The administrator will give you an admission form for hospitalization and will get in contact with your insurance company in order to get a coverage approval for your operation, or in some instances will get the insurance approval on your own.
You will then need to check with the hospital administration 3 days before your scheduled operation day to see if they have received your insurance approval.
Once your coverage insurance is approved, you should be present at the hospital 48 Hours prior to your operation day and submit to the Admission Office on level -1 the following documents:
- Admission Form for Hospitalization
- Insurance Approval in case you obtained your approval on your own
You will then be directed to complete your preoperative tests.

National Social Security Fund (NSSF):
You should present the following information and documents to the Scheduling Office.
- Inform the administrator that you are covered by the NSSF
The administrator will give you an admission form for hospitalization and a NSSF form that you need to present to the NSSF center in order to get your coverage approval from the NSSF.
Once you get your NSSF coverage approval, you should be present at the hospital 48 Hours prior to your operation and submit to the Admission Office on level -1 the following documents:
- Admission Form for Hospitalization
- NSSF Coverage Approval
- ID Card

Cooperative of Government Employees (COOP):
You should present the following information and documents to the Scheduling Office.
- Inform the administrator that you are covered by the COOP
The administrator will give you an admission form for hospitalization, a COOP form along with a medical prescription.
Then you need to present all these documents to the COOP center in order to get an approval for undergoing your operation.
Once you get your COOP coverage approval, you should be present at the hospital 48 Hours prior to your operation day and submit to the Admission Office on level -1 the following documents:
- Admission Form for Hospitalization
- COOP Coverage Approval

Army:
You should present the following information and documents to the Scheduling Office.
- Present your Pre-approved form from the Army center
- Army ID Card
The administrator will give you an admission form for hospitalization, a report confirming that you have been transferred from the Army Hospital to the Eye & Ear University Hospital and a Proforma if needed (depending on the type of operation)
Then you need to present all these documents to the Army center in order to get an approval for your operation( بطاقة تعهد و أخذ على العاتق)
You should be present at the hospital 48 Hours prior to your and submit to the Admission Office on level -1 the following documents:
- Admission Form for Hospitalization
- بطاقة تعهد و أخذ على العاتق
- Army Health Card (بطاقة صحية)
- Army ID Card

- Present your ISF ID

Then you are required to present all these documents to the ISF center in order to get an approval for undergoing your operation.
Once you get your ISF coverage approval, you should be present at the hospital 48 Hours prior to your operation day and submit to the Admission Office on level -1 the following documents:
- Admission Form for Hospitalization
- ISF Coverage Approval
- ISF Identification Card

The administrator will prepare for you an admission form for hospitalization, all MOH related documents and a Medical Prescription.
You should pick up all the above documents 4 days prior to your operation day.

Once you get your MOH coverage approval, you should be present at the hospital 48 Hours prior to your operation and submit to the Admission Office on level -1 the following documents:

Self-Pay:
You only need to schedule a day for your operation with the scheduling office. You will then be given an admission form.
You are requested to be present at the Hospital 48 Hours prior to your operation day to submit your admission form to the Admission Office on level -1 and settle your operation fees.
